孔武饰演冷酷的雷·布雷斯林（西尔维斯特·史泰龙饰）在精心策划后，成功从戒备森严的监狱越狱。他隶属于独立的BC安保公司，该公司受联邦监狱局的委托，负责测试全国监狱的安全性。它利用布雷斯林这样的越狱大师寻找监狱的漏洞，确保没有囚犯从笼子里逃脱。在过去的八年里，雷成功征服了14所监狱。这一次，为了验证关押凶恶恐怖分子的设施的安全性，中情局委托即将退休的雷进入神秘的国际拘留中心。但这次行动似乎出了问题，雷与小队失去了联系。冷酷无情的监狱长霍布斯（詹姆斯·卡维泽饰）也完全无视他撤离的请求。维尼·琼斯（维尼·琼斯饰）热衷于冷漠地对待人的生命。 孤立无援的雷只能在囚犯埃米尔（阿诺·施瓦辛格 Arnold Schwarzenegger 饰）的帮助下，寻找逃脱的机会……
